It means exactly what it says. The voltage doesn't swing high and low like it's supposed to - it stays at center. There's also a code for voltage stays low, and voltage stays high.

Yeah. Pouring raw fuel out the tailpipe is pretty rich man. I'd pop for a new O2. If it's still there, then you've more than likely got a wiring issue.
