I would urge you all to Complete that form and to submit it immediately. It will help to further our "combined" case if we show that it wasn't individuals who were scammed here, but a group of people. I have filled out the form and submitted it and I would ask that you all do the same. Since this IS a state government agency try and keep the content of the complaint somewhat professional and not belittle it by saying how you want to go to his house and smash his knees with a baseball bat.
I have all the relevant information needed to complete the form if any of you need it. Just contact me and I will forward it to you. I can also help you to write the complaint portion if necessary to present a unified and professional claim against Lee Johnston AKA mrslick.
