Well i have had a rattle that wouldnt go away so today i took the trans down and find that the flexplate ,around holes of clutch and bolts are f'ed up . The center cap on the clutch fell off as well . How exactly did this happen ? i have checked the bolts 2 times and could have sworn they were tight . Do you think its a good idea to use the clutch still with the marks in the pic ?

yeap, looks like they backed out.

The flywheel looks okay, as long as none of the threads are torn up. You'll need a new flexplate and new flexplate bolts, though.

great to know i can still use the clutch . The treads are in good shape just gouged around the hole . The middle cap of the clutch also fell off probably from the massive rattling that was goin on . Do i need that center cap ?

No you dont really need it. Its just there as a dust or debris shield.
